What this website collects

This site collects only what you type into the contact form: your name, email address, phone number, how you would like us to reply, a preferred day, and the message you write. The form asks for no medical details, and we ask you not to include any.

How we use it

Shard Dental uses it to answer your request and to arrange a visit. We do not sell your information.

Health records

Your health history and consent forms are not collected on this website. They are completed in the practice’s own secure portal or on paper at the office. We keep your records private and share them only as the law allows, for example with your insurance plan when you ask us to file a claim, with a dental laboratory making something for you, or with another provider when we refer you.

Cookies and tracking

This website sets no advertising or tracking cookies and does not use analytics tools.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to stop replying to you. Call the office or use the contact page.

Changes to this policy

If this policy changes, the new version replaces this page.
